I've been spending a lot of time answering questions about sleep as I see not many doctors are. Questions usually involve diagnosis and treatment of obstructive sleep apnea, insomnia and restless leggs syndrome. Medications are another issue that comes up a lot about what to take for sleep, what's helpful for sleep onset insomnia, what's harmful to those with sleep apnea, etc. With any form of insomnia, I always emphasize the need for good sleep hygiene.
Another topic that comes up frequently is patients who are already diagnosed with OSA and are on either CPAP or BiPAP therapy, but still not feeling well rested. Continuous positive airway pressure or Bilevel positive airway pressure are the gold standard treatments for sleep apnea. If you're already on therapy and still feeling excessive daytime somnolence and fatigue, you need to follow up with your sleep specialist. It's possible that the settings on the device are no longer effective in your treatment. A repeat sleep study may also be in order if it's been more than 3 years for a formal re-evaluation.
